What HRT can and cannot do

Menopausal hormone therapy is the most effective treatment for hot flushes and night sweats and can improve sleep disrupted by them. Systemic therapy helps prevent bone loss while it is used. Local vaginal estrogen treats genitourinary symptoms with much lower systemic exposure. HRT is not prescribed to prevent ageing or as a universal heart-disease treatment.

Build the prescription around the person

The plan depends on uterus status, age, time since menopause, symptom priorities, bleeding, migraine, blood pressure, diabetes, lipids, liver and gallbladder health, clot history and cancer history. Women with a uterus usually need progestogen with systemic estrogen. Transdermal routes may be preferred with metabolic or clot concerns.

When HRT may not be appropriate

Absolute or major contraindications include unexplained vaginal bleeding, current or previous estrogen-dependent cancer in many circumstances, active or recent venous thrombosis, stroke or heart attack, severe active liver disease and pregnancy. Complex histories require specialist input rather than a blanket yes or no.

Before and after starting

A good consultation covers symptom and period history, medicines, supplements, family history, blood pressure and age-appropriate screening. Routine hormone panels are often unnecessary in women over 45 with typical symptoms. Review benefit, side effects and bleeding after starting and at least annually. New heavy bleeding, chest pain, breathlessness, one-sided leg swelling or neurological symptoms needs urgent care.
