Signs of early menopause at 40: causes, tests and treatment

Irregular periods before 45 deserve a proper review. Learn the difference between early menopause and POI, what else can cause cycle changes, and why treatment protects more than comfort.

The short answer

Menopause before 45 is called early menopause. Loss of ovarian function before 40 is usually described as premature ovarian insufficiency, or POI. A change in periods at 40 can be perimenopause, but pregnancy, thyroid disease, high prolactin, major weight change, polycystic ovary syndrome, stress, medicines and uterine conditions can create similar patterns. Age should prompt assessment, not self-diagnosis.

The most useful first step is a timeline: first day of each bleed, flow, missed cycles, hot flushes, sleep, vaginal or urinary symptoms, medicines, contraception, pregnancy possibility, surgery, chemotherapy and family history. Bring the record to a gynaecologist or clinician rather than ordering a large hormone panel without context.

Possible signs

The common clue is a sustained change in menstrual pattern: cycles may become farther apart, unpredictable or stop. Hot flushes, night sweats, sleep disruption, mood change, brain fog, vaginal dryness, painful sex and urinary symptoms can occur. None of these alone proves early menopause. Some people with POI still ovulate intermittently, so symptoms and fertility can fluctuate.

Heavy bleeding, bleeding between periods, bleeding after sex, severe pelvic pain or anaemia symptoms should be investigated on their own merits. Early menopause does not explain every midlife symptom and should not become a reason to miss fibroids, endometriosis, cervical disease or another condition.

Diagnosis and useful tests

Testing is guided by age and history. It usually begins by excluding pregnancy and may include thyroid and prolactin testing. When POI is suspected, guidelines support FSH assessment on two samples taken several weeks apart rather than relying on one result. Oestradiol can support the clinical picture. Further genetic or autoimmune assessment may be appropriate in confirmed, unexplained POI.

AMH estimates ovarian reserve in selected fertility settings, but it is not a stand-alone menopause test and cannot predict the exact date of the final period. Hormonal contraception can alter bleeding and some test results. Tell the clinician exactly what you use before interpreting a number.

Why timely treatment matters

Earlier loss of ovarian hormones means more years of exposure to lower estrogen, which can affect bone and cardiovascular health. When medically suitable, guidelines commonly recommend hormone replacement until around the usual age of natural menopause, using HRT or in some situations a combined hormonal contraceptive. The purpose is symptom relief plus replacement of hormones the body would otherwise normally produce at this age.

Treatment is individual when there is a history of hormone-sensitive cancer, clotting, migraine, liver disease or unexplained bleeding. Bone health, blood pressure, lipids, glucose, smoking, movement, calcium, vitamin D and resistance exercise also belong in the plan. Early menopause care should not end with a prescription.

Fertility and emotional health

POI is not identical to irreversible sterility. Intermittent ovulation can occur, while spontaneous pregnancy is uncommon and unpredictable. Anyone wishing to avoid pregnancy still needs contraceptive advice. Anyone hoping to conceive deserves an early reproductive-medicine referral and honest discussion of options without guarantees.

An unexpected diagnosis can affect identity, relationships and plans for parenthood. Psychological support is part of appropriate care, not an optional extra. Ask for a written plan that covers treatment, review dates, fertility, contraception, bone protection and who to contact when symptoms change.

When to seek urgent care

Seek urgent help for chest pain, severe breathlessness, fainting, one-sided weakness, sudden severe headache or very heavy bleeding with dizziness. Book prompt assessment for a positive pregnancy test with pain, persistent abnormal bleeding, a new breast lump or severe depression. These symptoms need their own diagnosis rather than being attributed to hormones.

QUICK ANSWERS

Frequently asked questions

Is menopause at 40 normal?

It can occur, but it is earlier than average and deserves assessment and long-term health planning.

Can early menopause be reversed?

There is no proven treatment that reliably restores ovarian function, although POI can fluctuate.

Do I need two FSH tests?

When POI is suspected, repeated testing several weeks apart is commonly recommended.

Can I still become pregnant?

Intermittent ovulation can occur, so discuss both fertility and contraception.

How long is HRT used?

When suitable, replacement is often advised until around the usual natural menopause age, with regular review.
